Adult Day Care and Alzheimer's Day Care

Adult Day Care offers needed care for frail elderly in a supervised, protective setting during the day. Services include social and recreational activities, training essential for sustaining the activities of daily living, and a hot lunch. Services are provided in a licensed facility. Some programs provide services specific to health conditions as in the case of persons with Alzheimer's disease. Alzheimer's Day Care offer day care for persons with Alzheimer's disease (and other related dementia) who are often unable to be served by other programs.

Case Management

Case Management provides support to seniors and their caregivers, enabling seniors to maintain their maximum level of independence. Services include assessment of needs, coordination of needed services and monitoring and follow-up to determine the effectiveness of the services provided.

Multipurpose Senior Services Program (MSSP)

Multipurpose Senior Services Program (MSSP) coordinates a wide variety of services to help adults age 65 and older remain at home. Participants must be MediCal eligible and in frail health or at risk of going to a nursing home. MSSP is a service provided by the Area Agency on Aging.

General Information

Elder Abuse Prevention

Elder Abuse Prevention Project (EAPP) is a project of the AAA that includes education, training and court advocacy to increase awareness, promote prevention strategies and reduce the incidence of elder abuse in Sonoma County. The EAPP is comprised of concerned citizens, service providers, local and state government representatives, professionals in the legal and health care field and law enforcement personnel interested or involved in elder abuse prevention issues.
